On December 1, 2025, Michael Childers, a former BOP Lieutenant previously assigned to the United States Penitentiary Big Sandy (USP Big Sandy) in Inez, Kentucky, pleaded guilty in the Eastern District of Kentucky to one count of falsifying a document with intent to obstruct a federal investigation. According to the factual statement in support of the guilty plea, in April 2021, Childers falsified an incident report to cover up the assault of an inmate by BOP staff in the lieutenants’ office at USP Big Sandy. The investigation is being conducted by the OIG’s Lexington Locale and the FBI.
